What to Bring to Your DOT Exam
Thank you for scheduling your DOT physical exam. To help your appointment go smoothly and avoid delays, please bring the following items with you:
A valid driver's license or state ID
Your current Medical Examiner's Certificate (if you have one)
A complete list of your medications (including doses and prescribing providers)
Glasses or contact lenses (if used for driving)
Hearing aids (if worn)
Medical records for recent surgeries, heart conditions, diabetes, sleep apnea, or other chronic conditions
CPAP compliance report (at least 90 days of data)
Clearance letters from specialists (if previously required by DOT)
Note: Your urine sample will be collected in-office for the required urinalysis.
Please complete section one of the Medical Examination Report Form prior to your arrival.
